First visits are a great time to become familiar with our practice and skilled team of caring professionals. Upon your arrival, our patient coordinator will gather new patient forms, insurance and health information and provide an office tour. After a stress-free dental and gum examination is completed, Dr. Su will meet with you to discuss individualized treatment options for your child and answer any of your questions. We look forward to your first visit and will provide you with the necessary information and treatments to help your child maintain a lifetime of healthy smiles.

Being prepared for your child’s first appointment guarantees the best possible results. For your convenience, please complete the new patient and history form listed below before your appointment. Also, please bring your current insurance card, policy number and/or insurance forms to the first appointment.
